Weights supports custom exercises. Use them when a movement needs a distinct identity in your routine, and use notes for setup details that do not need a separate exercise.

Search before creating another name
Choose the existing movement when it accurately describes what you perform. Creating several names for the same exercise can split your history and make review harder. A custom entry earns its place when equipment, movement or variation makes the old name misleading. Decide on a name you will still understand after several months.
Build a simple naming convention
Start with the movement and add the variation or equipment: “Row — chest-supported dumbbell” is clearer than “Row 2.” Keep labels concise enough to scan between sets. Do not append today’s weight, repetitions or date to the exercise name; those details belong in the recorded sets and session history.
Separate identity from setup
An incline press and a flat press can deserve different exercise names. Seat height on the same machine can live in a note. That distinction lets you find the right movement while preserving context. When you deliberately change the name or recording convention, add an explanation so future comparisons do not depend on memory.
Keep the limits in mind
A custom exercise is an organizational tool. Creating a name does not establish an instructional video, a medical recommendation, automatic muscle classification or compatibility with another app’s exercise database. Use the in-app fields available to you and inspect the saved entry before relying on it. Routines and notes are complementary parts of the logging workflow.

Should I create a new exercise every time the weight changes?
No. Record weight changes in the sets so comparable performance stays under one exercise identity.
When is a note enough?
Use a note when the movement remains the same and you only need to remember a setup detail or explain a one-off change.
